Employees who are affected by COVID-19 should complete the COVID-19 Reporting Form under Campus Links on the Portal.

Financial Aid Forms

Forms are not required for all students. Please review your requested document list on MyOCCC Portal . Then submit requested forms  only. Be sure to complete all sections of requested form before submitting to prevent delays in your file review. Thank you!

Clarification of Program of Study Form

Continued Eligibility Form  All health programs have a selective admissions process. You must be accepted for admission to the health program before you submit a continued eligibility request. 

Suspension Appeal Form   Appeals must be submitted prior to the first day of class.
