Financial Aid Continued EligibilityĀ FormĀ All health programs have a selective admissions process. You must first be accepted for admission to the health program before you submit a continued eligibility request. You will receive an email of the decision within 2 to 4 weeks.
Financial Aid Suspension AppealĀ Form Ā Official transcripts from all colleges where you were previously enrolled must first be submitted directly to OCCC Records Office and evaluated for a financial aid appeal to be considered. If your appeal is not approved, you will not be eligible for federal grants or loans, so you must have another way to pay your courses. You will receive an email of the appeal decision within 2 to 4 weeks.
Financial Aid Change in GraduationĀ Form Use this form if you did not complete the degree requirements for your OCCC degree diploma and need to take/re-take additional courses.
