Authorization to Release Form with Proxy

If the Authorization to Release Form with Proxy is not signed in person, the student must provide additional documentation. Additional documentation includes a copy of a valid driver license, a valid state-issued ID card, or a passport. Electronic signatures are not permitted. No exceptions.
