Alternative Format Request Form
New students requesting alternative textbook formats from disability services at OCCC, must first contact Student Accessibility and Support at 682-7520 or email disabilityservices@occc.edu.
Students already authorized by OCCC disability services to receive alternative textbook formats may complete the online form for each book.
Policy
Students with disabilities who have difficulty with printed material may receive their textbooks in an alternative format. Eligibility for this accommodation is determined on an individualized basis, and students must be able to demonstrate a substantial visual or learning disability, which requires supplemental materials. In order to assure textbook availability, all relevant textbook information must be submitted to the Student Accessibility and Support office as soon as possible. Students are responsible for obtaining this information as soon as they register for classes. In general, 6-8 weeks notice is required to obtain alternative textbooks. The Office will make every attempt to provide material as promptly as possible. However, late requests will result in a delay in providing materials. Reasonable accommodations depend upon the nature and degree of severity of the documented disability. While the specific methods are requested by the student, it does not imply that a particular accommodation must be granted if it is deemed not reasonable and other suitable techniques are available.
Procedure and Student Responsibilities
- Determine which books you will require. This information can be obtained by contacting the Bookstore, or the department secretary for each course.
- Fill in the textbook information on the form - all lines must be completed.
- Provide a copy of each course syllabus as soon as it is available.
- Submit this form as soon as possible in order to assure textbook availability. A hard copy of this form may be requested by contacting Student Accessibility and Support at 682-7520.
