Applying for Admission

Fill out the Application for Admission and submit it via mail, fax, or in person to:
Print the Student Vaccination Status Form. THIS FORM IS REQUIRED TO ENROLL IN CLASSES. Students under the age of 18 must have their parent or legal guardian sign this form.

Regular Admission
High school curricular requirements, submission of academic credentials, and evaulation of academic preparation

Transfer Students
Admission requirements, course work, applicability towards degree requirements and analysis of transfer credits.

Health Programs
Special Admissions procedures for Nursing, Occupational Therapy Assistant, Physical Therapist Assistant, Surgical Technology and Respiratory Care Programs.

International Students
Application deadlines, admissions standards and procedures, fees, and finances and transfer of funds.

Students for whom English is a Second Language
English proficiency requirements, ESL Academic Bridge Program information, and resident alien admissions procedures.

Concurrent High School Students
High School Concurrent Approval Letter and requirements for highschool juniors and seniors, homestudy students, and students from unaccredited highschools.

Special Admission Categories
Non-degree seeking students, adult students, home study or unaccredited high schools, and opportunity admission.
