MILITARY CREDIT EVALUATIONS

To request credit for BASIC TRAINING, submit the following forms:

Army -One of the following: AARTS transcript, Copy of DD214, or Form 295
Navy / Marines - One of the following SMART transcript, Copy of DD214 or Form 295
Air Force -An official transcript from Community College of the Air Force.
Coast Guard - Copy of DD214 or From 295

NOTE: Credit for basic training does not satisfy the "Wellness & Positive Lifestyle" requirement.
To request credit for a FORMAL MILITARY SCHOOL, submit the following:
- AIR FORCE - An official transcript from the Community College of the Air Force. Write to:
  CCAF Registrar
  CCAF/RRR
  Maxwell AFB, AL 36112-6655
- ARMY / ARMY NATIONAL GUARD / ARMY RESERVES with basic active service dates falling on or after October 1, 1981, need to submit an AARTS transcript. Send a written request including your SS#, basic active service date, and complete address to:

  or visit their website at: aartstranscript.leavenworth.army.mil/
  
  (If your active duty dates do not fall within the AARTS time span, you will need to submit a completed Evaluation of Military School Training form, along with a copy of a certificate of completion for each course.)
- COAST GUARD - need to submit a completed Evaluation of Military School Training form, along with a copy of the certificate of completion for each course.
- NAVY / MARINES - submit a SMART transcript - to request transcript:
