You can report concerns about a student's progress with the Early Alert Form. Referrals should be made as soon as possible, but may be made at any time. This link can be downloaded to your desktop.

1. Remove any existing desktop shortcuts for this link
2. Right click on form
3. Highlight and click on "Create Shortcut"
4. Select "Yes" This creates a desktop shortcut that will directly access the "Faculty/Staff Referral" form
